checks, cash, Visa, MasterCard, and American Express, private insurance, and verified workers' compensation insurance
as remuneration for treatment rendered. The practice is also provider
for many managed care programs. The helpful staff would be glad
to investigate participation in your plan. Just have your insurance
card handy and give them a call. Applicable deductible amounts
and/or co-payments, whether fixed amount or a percentage of charges,
are due at each visit.
In order to utilize your insurance coverage optimally, you need
to be familiar with and investigate requirements for referral
for physical therapy. Most insurance carriers require a doctor’s
referral and a written prescription. Your policy may require this
to be your primary care doctor and may need preauthorization/
precertification. Note that any plan coverage quotes given by
your insurance carrier over the telephone are not a guarantee
of payment.
Policy provisions will be applied when a claim for services rendered
is filed. There may be limitations regarding length of treatment
(start date to end date), number of visits, treatment per diagnosis,
or noncovered treatment. It is highly recommended that you review
your policy booklet and contact your insurance carrier with any
questions you may have prior to treatment with any specialist.
It is helpful to jot down the date, time, and the name of the
person you spoke with should questions arise at a later date.
